Community Alternative Program
A Community Program for Aged and Disabled Adults.
Information:
Participants in this program must be deemed appropriate for intermediate or skilled nursing care by a physician and have needs that can safely be met at home.
Services:
Services include Case Management, In-Home Aide Services, Telephone Alert, some medical supplies and some medical equipment.
Eligibility:
To be eligible an individual must be deemed appropriate for intermediate or skilled nursing In-Home Care by a physician. They must need assistance with personal care, health needs, and household tasks, and have a responsible care giver. This program requires that the individual has needs that can safely be met at home.
