Community Alternative Program

A Community Program for Aged and Disabled Adults

Information

Participants in this program must be deemed appropriate for intermediate or skilled nursing care by a physician and have needs that can safely be met at home.

Services include:

Case Management
In-Home Aide Services
Telephone Alert
Some medical supplies
Some medical equipment

Eligibility

To be eligible, an individual must be deemed appropriate for intermediate or skilled nursing in-home care by a physician. The individual must need assistance with personal care, health needs, and household tasks, and must have a responsible caregiver. The program also requires that the individual’s needs can safely be met at home.

Aging Program

Provides a Limited Number of Hours of In-Home Care

Information

The Aging Program provides a limited number of hours of In-Home Aide Assistance to help individuals remain in their homes while maintaining their health and dignity.

Services include:

Bathing, dressing, and toileting
Meal preparation
Light housekeeping

Eligibility

To be eligible, an individual must be age 60 or older, need assistance with personal care and household activities and have no responsible person available to provide the needed care.

Personal Care Program

Provides an In-Home Aide for a Maximum of 60 Hours per Month

Information
The Personal Care Program provides a limited number of hours of In-Home Aide assistance to help individuals remain at home while maintaining their health and dignity.
A physician must refer the individual to an independent assessor in Raleigh. Once the referral is received, a nurse will conduct an in-home assessment. The number of hours of care approved will be determined based on that assessment.

Services include:

Bathing, dressing, and toileting
Meal preparation

Eligibility
To be eligible, an individual must:

Be eligible for regular Medicaid
Have a medical condition that requires direct, ongoing care prescribed by a physician
Need assistance with personal care tasks due to a medical condition
Be medically stable

Private Pay Program

Assistance in the Home for Individuals and Their Families

Information
Care services include meal preparation, transportation, light housekeeping, personal care, and respite care for aged or disabled adults.

Services include:

Sitter services
Light housekeeping
Personal care
Respite care
Transportation services

Eligibility
An in-home assessment is required to determine the individual’s needs.

Respite Care Program

Designed to Give Caregivers a Break from Daily Care

Information
This program provides temporary In-Home Aide services to eligible clients, allowing caregivers a break from the daily care they provide to their loved ones. The number of hours available is determined by the amount of grant funding available.

Services
In-Home Aides can assist with:

Personal care such as bathing, dressing, and feeding
Light housekeeping

Eligibility
To be eligible for this program, the individual receiving care must:

Be 60 years of age or older, or
Have dementia, and
Not be eligible for other services
